Team Lead- Full Time

Supervisor of Community Networkers and Overnight Support Staff. Team leaders also provide direct support of individuals with developmental disabilities.

Schedule: Monday and Tuesday 3p-11p, Wednesday 3p-9p, and Friday 3p-11p + In-office hours. Location is ISL-KS.

Service Coordinator- Full Time

Responsibilities
Organize the delivery of services and supports for persons with MR and/or developmental disabilities, belong to and provide leadership as necessary to a planning team that includes the individual, their family and service providers.
Link the available resources to assist the individual and family to meet the identified needs.

Provide case management, which includes four basic activities: connecting with individuals, planning for services, linking individuals to services, and advocating for service improvement.

Employ Person-Centered Thinking at all times as defined in the Medicaid Waiver Certification Principles, as established by the Department of Mental Health.

Qualifications

Four year Bachelor's Degree in human service or related field, plus two year's post graduate experience working with individuals with developmental disabilities.

Community Networker - Full Time

Direct support personnel working directly with individuals who have developmental disabilities. Your role is that of a mentor, teacher, peer, and advocate. Your responsibility is to support individuals with achieving their life dreams and goals, and to assist each person in achieving and maintaining what makes sense in their life. To be successful with this job you must be: respectful, a good listener, able to think quickly, make adaptations as needed, and maintain a clear sense of priority. A sense of humor is always useful. While you are on the job, an individual's health, safety and welfare are your responsibility. You will also be responsible for responding to any emergency needs, illnesses, and/or personal care that an individual may require.
