Service Coordination
- Who We Are, Who We Serve and What We Do
In
June 2008, Progressive Community Services began providing
service coordination to all developmentally disabled Buchanan
County residents who are found eligible for services by the
Missouri Department of Mental Health, Division of Developmental
Disabilities.
Service Coordinators are professionals who help people with
developmental disabilities and their families identify and
obtain needed services and supports. They also advocate for,
monitor, and evaluate services along with the individuals,
families and guardians. A key role of the Service Coordinator
is to assist the individuals they support with the process
and paperwork necessary to obtain services. The system of
multiple agencies providing services to persons with developmental
disabilities can be complex and confusing. Service Coordinators
provide a “single point of entry” into services.
Progressive Community Services currently serves approximately
500 individuals. Each Service Coordinator maintains a caseload
below forty individuals in order to provide personalized,
quality service coordination.
Once the intake has been completed by ARO, and eligibility
is determined the referral will be made to PCS for a Service
Coordinator. Upon referral and assignment of a Service Coordinator,
the individual and/or family guardian will be contacted within
24 hours to begin service coordination. The job of a Service
Coordinator is to meet and talk with the individuals that
we serve, their families / significant others and service
providers to gather information to develop each person’s
Individual Plan.
Service Coordinators assist in indentifying the individual’s
strengths and abilities, life goals, as well as service and
support needs. The Service Coordinators advocate for those
needs to be met through a variety of formal and informal providers.
Service Coordinators locate resources, make referrals, monitor
services that are being provided and help assure that the
person’s Individual Plan is being implemented.

Residential Services
Providing
Quality Living Arrangements for Adults with Developmental
Disabilities
Progressive
Community Services offers several different kinds of residential
services in the community that are designed to provide person-centered
supports to adults with developmental disabilities to promote
independence and ensure a safe, healthy environment.
In-Home Supports
Many individuals with developmental disabilities prefer
to live in the homes of family members. In-Home Residential
Services help meet a person’s or a family’s support
needs in order to allow the person to continue living in the
family home. Services are based on individual needs and can
range from assistance with personal care to skill development
to help accessing community activities.
Group Homes
Group Homes are an option for individuals who enjoy living
with four to five other adults. Support is provided through
24-hour staffing. Progressive Community Services has two group
homes located in St. Joseph. Residents are encouraged to participate
as members in their local community. Support provides individuals
with experiences that maximize opportunities and facilitate
community integration.
Individualized Supported Living
Through this residential service option, two or three individuals
who receive services rent and share an affordable home or
apartment located in their natural community. Services and
support staff arrangements are based on individual needs.
While some people may need and prefer 24-hour staffing, others
may use less support. Flexibility is important and allows
people to be in control of their own life situation.
